Understanding this fundamental concept is essential for anyone working in food safety, as it forms the foundation for preventing and managing food-borne illnesses that affect millions of people worldwide.

What is a disease?
At its core, disease represents a deviation from the normal state of an organism. More specifically, disease refers to any harmful change in the tissues or metabolic processes of living organisms that results in observable symptoms and functional impairment. These changes disrupt the delicate balance that organisms maintain to stay healthy.
Disease involves the breakdown of homeostatic control mechanisms that normally keep our bodies functioning properly. When these control systems fail, whether due to external agents or internal factors, disease occurs. The body’s normal chemical, physical, and functional processes become disrupted, leading to illness.
The components of disease
Understanding disease requires knowing three key components. First, the etiology refers to the cause of the disease. Second, pathogenesis describes the stepwise progression of how the disease develops in the body. Third, the clinical manifestations are the actual symptoms that patients experience and the signs that medical professionals observe.
Diseases present themselves through two main ways. Symptoms are what patients complain about and feel, such as pain, nausea, or fatigue. Signs are what trained observers can measure and detect, like fever, inflammation, or laboratory test abnormalities. Together, these help identify and diagnose specific diseases.
What are pathogens?
While not all diseases are caused by external agents, many of the most concerning food-borne illnesses result from pathogens. Pathogens are microorganisms that cause disease by entering the body and disrupting normal functions. These microscopic invaders include bacteria, viruses, fungi, protozoa, and even infectious proteins called prions.
It’s important to note that most microorganisms are not harmful. In fact, many are beneficial and necessary for our health. However, when disease-causing pathogens enter areas of the body where they shouldn’t be, they can multiply and cause significant harm.
Bacteria as disease-causing agents
Bacteria are single-celled organisms that obtain nutrients from their environments. While many bacteria help our bodies function properly, particularly in the digestive system, pathogenic bacteria can cause serious illnesses. In food safety, bacteria such as Salmonella, Campylobacter, and E. coli are major concerns.
These bacterial pathogens affect millions of people annually, causing symptoms including fever, headache, nausea, vomiting, abdominal pain, and diarrhea. Unlike viruses, bacteria can reproduce on their own and often survive on surfaces for extended periods, making food contamination a persistent challenge.
Viruses and their impact
Viruses differ significantly from bacteria. They are much smaller and consist merely of genetic material wrapped in a protein coating. Viruses cannot reproduce on their own and must invade host cells to multiply. Once inside your cells, they hijack the cellular machinery to create more viruses, often damaging or destroying the host cells in the process.
Common food-borne viruses include norovirus and Hepatitis A. These viruses can spread through contaminated food or water, causing illness ranging from acute gastroenteritis to long-term liver damage.
Fungi, yeasts, and molds
Fungi represent another category of pathogens relevant to food safety. These organisms reproduce mainly through spores and can be either microscopic or quite large. In food contexts, fungi cause problems in two main ways. First, certain fungi can directly infect humans, causing diseases like histoplasmosis or candidiasis. Second, some fungi produce dangerous toxins called mycotoxins in contaminated food.
Mycotoxins can contaminate staple foods like corn and cereals. Long-term exposure to these toxins can damage the immune system, interfere with normal development, and even cause cancer.
Protozoa as parasitic pathogens
Protozoa are single-celled organisms that don’t have cell walls and reproduce mainly by splitting in two. These parasites need moisture to thrive and often spread through contaminated food or water. Examples of protozoal diseases include malaria, giardia, and toxoplasmosis.
Some protozoa form protective cysts that allow them to survive harsh conditions outside the body for long periods, making them particularly challenging to eliminate from food and water supplies.
Prions: unusual infectious proteins
Perhaps the most unusual category of pathogens are prions. Prions are infectious agents composed of protein associated with neurodegenerative diseases. Unlike other pathogens, prions aren’t living organisms. They’re misfolded proteins that can cause other normal proteins to misfold as well, leading to devastating brain diseases like mad cow disease and variant Creutzfeldt-Jakob disease.
How diseases affect the body
When pathogens enter the body, they cause disease through various mechanisms. Some bacteria release toxins that directly poison cells and tissues. Viruses destroy host cells as they replicate. Other pathogens trigger excessive immune responses that damage the body’s own tissues. All these processes result in the harmful changes to tissues and metabolism that define disease.
The severity of disease depends on multiple factors, including the type and number of pathogens, the individual’s immune system strength, and whether the person has underlying health conditions. In food-borne diseases, factors like the amount of contaminated food consumed and how quickly treatment begins also play crucial roles.
The food-borne disease connection
Food safety statistics reveal the massive scale of food-borne diseases. Approximately 600 million people become ill from eating contaminated food each year, and 420,000 people die annually. Children under five years old are especially vulnerable, carrying 40% of the food-borne disease burden despite being a much smaller portion of the population.
Food-borne illnesses occur when people consume food or beverages contaminated with pathogens or their toxins. Microorganisms that cause disease are called pathogens, and when they enter the food supply, they can cause serious health problems. Common food-borne bacterial pathogens include Salmonella in eggs and poultry, Campylobacter in raw milk and undercooked poultry, and E. coli in undercooked meat and contaminated produce.
Prevention through understanding
Understanding what constitutes a disease and how pathogens cause illness is fundamental to food safety. This knowledge enables food handlers, producers, and consumers to take appropriate preventive measures. Proper cooking destroys most pathogens, adequate refrigeration prevents their multiplication, and good hygiene practices prevent contamination in the first place.
The economic impact is staggering too. Low- and middle-income countries lose approximately $110 billion annually in productivity and medical expenses due to unsafe food. These diseases strain healthcare systems and harm economies, tourism, and trade.
Moving forward with food safety
Disease is not simply about feeling unwell. It represents a complex disruption of the body’s normal state, often caused by pathogens that hijack our biological systems for their own survival. In the context of food safety, understanding disease means recognizing that harmful microorganisms can contaminate food at any point from farm to table.
This understanding empowers everyone in the food chain to take responsibility. Governments can establish better food safety regulations. Food producers can implement stronger quality controls. Consumers can practice safer food handling at home. Each action, informed by knowledge of disease and pathogens, contributes to reducing the burden of food-borne illnesses.
