Food moves across borders faster than ever before. A shrimp farmed in Thailand might end up on a dinner plate in Germany within days. This global food supply chain brings convenience and variety, but it also introduces risks. When food safety fails, the consequences spread quickly. The World Health Organization (WHO) stands at the center of efforts to establish international health standards that protect consumers worldwide-particularly in the realm of food safety.

The scale of the food safety challenge

Unsafe food is not a minor inconvenience-it represents a significant global health threat. According to WHO, contaminated food containing harmful bacteria, viruses, parasites, or chemical substances can cause more than 200 different diseases, ranging from diarrhoea to cancers. The numbers are staggering: approximately 600 million people-nearly one in ten globally-fall ill after eating contaminated food each year. This results in 420,000 deaths annually and the loss of 33 million healthy life years.

Children bear a disproportionate burden of this crisis. WHO estimates indicate that children under five years of age carry about 40% of the foodborne disease burden, with 125,000 children dying from these illnesses every year. The impact extends beyond health-the World Bank has estimated that unsafe food costs low- and middle-income countries US$110 billion annually in lost productivity and medical expenses.

How WHO shapes global food safety standards

WHO’s influence on international food safety operates through several interconnected mechanisms. The organization does not simply issue guidelines and hope countries follow them. Instead, it has built an infrastructure of scientific advisory bodies, international standards, and legal frameworks that shape how nations approach food safety.

The Codex Alimentarius Commission

The cornerstone of WHO’s food safety work is the Codex Alimentarius Commission, a joint initiative with the Food and Agriculture Organization (FAO). Established in 1963, the Commission develops what is essentially the global rulebook for food safety. The Codex Alimentarius-Latin for “Food Code”-is a collection of international standards, guidelines, and codes of practice designed to protect consumer health and ensure fair practices in food trade.

These standards carry significant weight in international commerce. The World Trade Organization recognizes Codex standards as the international reference point for food safety under its Agreement on Sanitary and Phytosanitary Measures. When trade disputes arise over food safety concerns, Codex standards often serve as the benchmark for resolution.

The Commission’s membership reflects its global reach. Currently, it comprises 188 member countries, one member organization (the European Union), and more than 230 observers from intergovernmental and non-governmental organizations. This broad participation ensures that standards reflect diverse perspectives and can be implemented across different economic and cultural contexts.

Scientific advisory bodies

What distinguishes WHO’s food safety work is its foundation in rigorous science. The organization manages several expert committees that provide the scientific evidence underpinning Codex standards:

Joint FAO/WHO Expert Committee on Food Additives (JECFA) – Established in 1956 (predating the Codex Commission itself), JECFA evaluates the safety of food additives, contaminants, naturally occurring toxicants, and residues of veterinary drugs in food. When you see a maximum permitted level for a food additive on a label, JECFA’s assessments likely informed that number.

Joint FAO/WHO Meeting on Pesticide Residues (JMPR) – This group assesses pesticide residues in food and establishes maximum residue limits that protect consumers while allowing for practical agricultural use.

Joint FAO/WHO Expert Meetings on Microbiological Risk Assessment (JEMRA)This body provides scientific advice on microbiological hazards in food, informing guidelines on everything from handling raw poultry to preventing contamination in ready-to-eat foods.

Measuring the problem: burden of foodborne disease

Effective policy requires reliable data. In 2015, WHO published the first-ever global estimates of the burden of foodborne diseases, a landmark achievement that quantified what had previously been poorly understood. The Foodborne Disease Burden Epidemiology Reference Group (FERG) examined 31 foodborne hazards and found they were responsible for 600 million cases of foodborne illness in 2010.

Research published in PLOS Medicine revealed that the global burden of foodborne disease is comparable to that of major infectious diseases like HIV/AIDS, malaria, and tuberculosis. The most frequent causes were diarrheal disease agents, particularly norovirus and Campylobacter species. Deaths were most commonly caused by non-typhoidal Salmonella, Salmonella Typhi, Taenia solium, and hepatitis A virus.

Perhaps most significantly, the research demonstrated that the burden falls disproportionately on low-income regions and vulnerable populations. Children under five-representing just 9% of the global population-bear a substantial portion of this disease burden. This disparity drives WHO’s focus on building food safety capacity in developing countries.

The World Health Assembly recognized the importance of ongoing monitoring through Resolution WHA73.5, which mandated WHO to regularly report on the global burden of foodborne diseases. Updated estimates are expected in 2025, providing fresh data to guide policy interventions.

From science to practice: WHO’s Five Keys to Safer Food

International standards are meaningless if they cannot be translated into everyday practice. WHO bridges this gap through practical guidance like the Five Keys to Safer Food, a public health campaign that distills food safety science into actionable steps. The core messages are straightforward: keep clean; separate raw and cooked foods; cook thoroughly; keep food at safe temperatures; and use safe water and raw materials.

The poster communicating these principles has been translated into more than 87 languages, making it one of WHO’s most widely disseminated food hygiene resources. The simplicity is intentional-these principles address the common handling errors that cause most foodborne illness outbreaks worldwide.

WHO data indicate that a small number of food handling errors account for most foodborne disease episodes globally. These include preparing food hours before consumption and storing it at temperatures that allow bacterial growth, insufficient cooking or reheating, and cross-contamination between raw and cooked foods.

While Codex standards are voluntary guidance documents, WHO does possess a legally binding instrument that covers certain aspects of food safety. The International Health Regulations (IHR) represent a binding agreement for all 194 WHO Member States. These regulations aim to help the international community prevent and respond to acute public health emergencies that could cross borders-including emergencies caused by contaminated food.

Food safety emergencies can spread rapidly through international trade. A contamination event in one country can affect consumers across multiple continents before the source is even identified. The IHR provides a framework for countries to share information and coordinate responses to such events, making it a critical component of global health security.

The WHO Global Strategy for Food Safety 2022-2030

WHO’s current strategic framework for food safety envisions a world where all people, everywhere, consume safe and healthy food to reduce the burden of foodborne diseases. The strategy emphasizes building strong, sustainable, and resilient national food control systems.

Key elements include strengthening national food legislation to align with Codex standards, developing evidence-based food safety risk management systems, and building laboratory capacity for food contamination monitoring. The strategy recognizes that food business operators bear primary responsibility for producing safe food, while governments must establish the regulatory framework and enforcement mechanisms.

The strategy also acknowledges emerging challenges. Climate change affects the geographic distribution of food safety hazards, altering patterns of pathogens, mycotoxins, marine biotoxins, and heavy metal contamination. Antimicrobial resistance poses growing concerns for both human health and food production systems. These evolving threats require adaptive responses.

Collaboration and partnerships

WHO does not work in isolation. The partnership with FAO is foundational, with complementary mandates-WHO typically oversees relationships with the public health sector while FAO addresses food safety along the production chain. Together, they cover the full spectrum from farm to fork.

The International Food Safety Authorities Network (INFOSAN) connects food safety authorities from around the world, enabling rapid information sharing during food safety emergencies. The FAO/WHO Codex Trust Fund helps developing countries participate effectively in international standard-setting, ensuring that global food safety standards reflect the realities faced by all nations.

World Food Safety Day, observed annually on June 7, brings together WHO, FAO, governments, food industry stakeholders, and consumers to raise awareness and drive action on food safety issues. These collaborative efforts recognize that food safety requires coordination across sectors and borders.

Looking ahead

The global food system continues to evolve. New food products, production methods, and distribution channels emerge constantly. Climate change reshapes agricultural patterns and disease ecology. Population growth and urbanization transform dietary habits and food supply chains. Through it all, WHO’s role in establishing and updating international health standards remains essential.

The organization’s approach-combining rigorous science, international consensus-building, practical guidance, and legal frameworks-provides a model for addressing complex global health challenges. Food safety may not generate headlines like pandemic outbreaks, but the daily toll of foodborne illness demands sustained attention and coordinated action.
